Patients wishing to register with the Practice will be asked to complete a New Patient Registration Form. You do not need proof of address or immigration status, ID or an NHS number.
New patients will be asked to attend for a New Patient Medical Examination. This will help the Practice to record information about your health and medical history which could be important if you needed to see a GP before your old medical records become available.
To register you will need to complete the New Patient Registration Form (PDF, 85KB), the NHS Zero Tolerance letter (PDF, 16KB) and the GMS1 Form, available from the Practice or available to download above. Alternatively Click here to complete the New Patient Registration form online
We ask that patients wishing to register bring their completed forms to the Practice after 12.30pm Monday to Friday for Reception to process the forms, when we are less busy.
